• Where Spiderflower documents work through photography or film, individual permissions are always sought from those appearing in the material and  consent is requested for photographs or film to be shared.

 

• Participants will be contacted if Spiderflower needs a permissions form to be completed.

• Simply fill in the form below and submit.  Spiderflower will confirm receipt via email.

• Permissions are kept for a period of 5 years from the date submitted, and are then subject to review.  Spiderflower will contact participants at the end of this period.

• Should a participant change their mind at any point in time, please contact Spiderflower to withdraw permission.
