​​Writing Advice: Where long answers are required, write a draft in a place where you can save, then copy and paste into the form as information will be lost if the page refreshes.

Multiple Events:  Please submit one form per project.  You can submit this form as many times as you wish.  If you have facilitated at multiple venues for a single project - include all venues, their contact details and dates for that venue in YOUR PROJECT / EVENTS section on the second page of the form.

Copies:  Keep a copy of your completed submission. Spiderflower will also forward on a copy of your submission to you once received.​

Confirmation:  Spiderflower will confirm your submission in person within 24 hours - please be in touch if you do not hear by then.​
